Thank you for your interest in joining Ilfracombe Dental Practice. We pride ourselves on the quality of dentistry we deliver to our patients, with treatments ranging from preventative to cosmetic in nature.

We have compiled this section for new patients to help set our registration process and highlight some useful information.

Useful information.

We have found that when some patients book their first appointment with us, they have already decided on the type of patient they want to be (e.g. NHS, Denplan, private). However, these choices can limit the types of treatment available to you. For example, an increasing number of NHS patients are now requesting tooth whitening, but this treatment is not provided on that scheme.

That’s why we like to take a consultative approach from the very start. Rather than pigeonholing our patients, we will ask about the type of treatment and service you require. Is it functional dentistry you are most interested in, or are you considering the cosmetic/aesthetic side too? Together, we can decide on the best model to fit your needs.

How to join the practice

  • Please complete a declaration of interest form, which you can either collect from our reception or download from this page. This PDF form is editable in most browsers (but not on Apple devices such as iPads or iPhones), so you can complete it on your computer or tablet.
  • Print the form, sign by hand, and bring it into reception. Do not try to email this if it is not sufficiently encrypted.
  • We will contact you when the practice can offer you an appointment. Please note that as a result of recent events, we would like to remind all that;
  1. Going on the waiting list does not mean you have joined the practice.
  2. Only completed forms will be accepted to our list.
  3. The practice will only consider offering you regular and emergency appointments after your first appointment with a dentist.
  4. Failure to arrive at your first appointment without giving 48Hrs prior notice to the practice that you cannot attend, will result in you losing your place in the waiting list.
  5. Your place in the waiting list or any regular/treatment appointments is not interchangeable with anyone else. Anyone attempting to impersonate anyone else in order to access treatment will result in both parties being removed from the practice.
  6. We enforce a “zero tolerance” policy to any form of harassment, abuse or demeaning behaviour to any members of our staff including whilst you are on the waiting list.
  • • On the day of your appointment, we request that you arrive 10 minutes early to complete our medical questionnaire and check your evidence of identification. You can download the medical history questionnaire form, as you did with the declaration of interest form, and complete it prior to your appointment.

Please don’t worry if you are having difficulty printing any of our forms as we have copies available at reception. Our dental team are diligently working as fast as possible, that uncompromised safety and care allows, to keep waiting times down.
