Thank you for your interest in joining Ilfracombe Dental Practice. We pride ourselves on the quality of dentistry we deliver to our patients, with treatments ranging from preventative to cosmetic in nature.
We have compiled this section for new patients to help set our registration process and highlight some useful information.
When some patients book their first appointment with us, they have already decided on the type of patient they want to be (e.g. NHS, Denplan, private). However, these choices can limit the types of treatment available to you. For example, an increasing number of NHS patients are now requesting tooth whitening, but this treatment is not provided on that scheme.
That’s why we like to take a consultative approach from the very start. Rather than pigeonholing our patients, we will ask about the type of treatment and service you require. Is it functional dentistry you are most interested in, or are you considering the cosmetic/aesthetic side too? Together, we can decide on the best model to fit your needs.
Our registration process
- To register with us, you will need to request an appointment either by phone or via the “Book an appointment” option on this page. It would help if you could indicate the services you are interested in, so we can begin tailoring our care to suit your needs.
- We will ask you to complete a registration form, which you can collect from reception or download on this page. This PDF form is editable in most browsers (but not on Apple devices such as iPads or iPhones), so you can complete it on your computer or tablet. Please then print it off, sign by hand and bring it into reception. Only complete registration forms will be accepted for processing for our waiting list. Using the information you have given us, we will contact you when the practice can offer you an appointment.
- On the day of your appointment, we request that you arrive 10 minutes early in order to complete our medical questionnaire. Alternatively, you can download the form, as you did with the registration form, and complete it prior to your appointment.
Please don’t worry if you are have difficulty printing any of our forms as we have copies available at reception.
